Improvements reported in muscle strength, fatigue and quality of life.

Vitamin D supplements may help those with Crohn’s disease overcome the fatigue and decreased muscle strength associated with the inflammatory bowel disease, according to new research.

Extra vitamin D “was associated with significantly less physical, emotional and general fatigue, greater quality of life and the ability to perform activities of daily living,” said Tara Raftery, a research dietitian and doctoral candidate at Trinity College Dublin. She is scheduled to present the findings Saturday at the Digestive Disease Week meeting in Orlando, Fla.

Raftery and her colleagues evaluated 27 patients who had Crohn’s in remission. (Even in remission, fatigue and quality of life can be problematic.) The patients were assigned to take either 2,000 IUs (international units) of vitamin D a day or a dummy vitamin for three months.

Before and after the study, the researchers measured hand-grip strength, fatigue, quality of life and blood levels of vitamin D.

“Hand-grip strength is a proxy measure of muscle function,” Raftery said. “Muscle function has been known to be reduced in Crohn’s disease.”

Besides boosting bone growth and remodeling, vitamin D is thought to improve neuromuscular and immune function, reduce inflammation and help with other bodily tasks. Children and adults aged 1 year to 70 are advised to get 600 IUs a day; older adults, 800, according to the U.S. National Institutes of Health (NIH).

Vitamin D is found in fatty fish such as salmon, in smaller amounts in cheese, egg yolks and beef liver, and in fortified foods such as milk.

Sometimes called the sunshine vitamin, vitamin D is also produced when the sun’s rays strike the skin.

Crohn’s can affect any part of the gastrointestinal tract, but most commonly affects the end of the small bowel and the beginning of the colon. Symptoms vary, but may include persistent diarrhea, rectal bleeding, abdominal cramps, and pain and constipation. About 700,000 Americans are affected, according to the Crohn’s & Colitis Foundation of America.

Its cause is not well understood, but Crohn’s is thought to involve heredity and environmental factors. Experts believe that in those with Crohn’s, the immune system attacks harmless intestinal bacteria, triggering chronic inflammation and, eventually, the disease symptoms.

The daily vitamin D supplement benefitted participants in many ways, Raftery found. “When levels of vitamin D peaked at 30 ng/mL (75 nmol/L) or more [a level considered healthy], muscle function in both the dominant and non-dominant hands were significantly higher than in those who had levels less than 30 ng/mL,” she said.

Quality of life improved more for the D-supplement group, too. Using a standard measure to evaluate quality of life, the researchers found those who achieved a healthy blood level of the vitamin scored 24 points higher than those not on supplements. A 20-point difference is considered meaningful from a “real-world” perspective, Raftery said.

Raftery now is testing vitamin D in a larger, year-long study of 130 Crohn’s patients.

The study results echo those of other researchers, including John White, professor of physiology at McGill University, Montreal. He said the research findings “show collectively that vitamin D acts in the intestine to stimulate the innate immune system to defend against pathogenic bacteria, and to enhance the barrier function of the intestinal epithelium [the lining of the intestine].”

Other researchers, including Raftery, have also shown vitamin D can help improve muscle strength, he said.

Vitamin D is getting a lot of attention in inflammatory bowel disease treatments, said Dr. Neera Gupta, co-chair of the Crohn’s & Colitis Foundation of America’s pediatric affairs committee.

More study is needed to determine the benefits of maintaining vitamin D levels higher than currently recommended, she said.

Gupta cautioned those with Crohn’s not to self-dose with vitamin D. “Discuss your vitamin D status with your primary gastroenterologist to determine whether or not vitamin D supplementation is indicated in your particular situation,” she said.

White said supplements are inexpensive and safer than too much sun exposure. A daily intake of 2,000 IUs is considered safe, he said. The safe upper limit for adults is 4,000 IUs, according to the NIH.

The data and conclusions of research presented at medical meetings should be viewed as preliminary until published in a peer-reviewed journal.

Stroke survivors who regularly go for brisk walks often report improved quality of life, according to a new study.

WEDNESDAY, March 7, 2013 — After stroke, physical activity is important to not only to regain motor and cognitive function, but to also help ward off another stroke.

In a new study, published in the journal Stroke, researchers found that stroke patients who regularly went for brisk walks saw  significant improvement in their quality a life — underscoring the findings and recommendations of previous research that has come to similar conclusions.

“Walking is a great way to get active after a stroke,” Carron Gordon, Ph.D., lead author of the study and a lecturer in the physical therapy department at University of the West Indies in Jamaica, said in a statement. “It’s familiar, inexpensive, and it’s something people could very easily get into.”

Researchers split 128 stroke survivors into two groups; one group walked briskly three times a week for three months, and one received therapeutic massage but did no supervised exercise. Gordon and his colleagues found that compared the therapeutic massage group, the walking group reported a 16.7 percent improvement in quality of life. In addition, they were able to walk 17.6 percent farther in a six-minute endurance test and had a 1.5 percent lower resting heart rate.

The results of the study are in line with previous research, which has found similar benefits to walking after stroke. Last month, researchers from the Nagoya University Graduate School of Medicine in Japan published a study that found that stroke patients who walked 6,000 steps or more per day were significantly less likely to have a second stroke or heart attack. They presented their research at the 2013 International Stroke Conference.

And even though 6,000 steps is approximately three miles, a distance some stroke patients may see as daunting, Yamada said it’s important for them to make the effort so they’ll not only feel better, their health will improve.

“It’s safe, it’s possible, and it’s even good for their health to take 6000 or more steps a day,” he said.

Stroke survivors are often wary of walking or exercising due to fears of falling — a fear that can lead to them being stuck in the house, Gordon said in the statement. He suggested that a family member or friend walk with the stoke patient until they are comfortable enough walking alone — as in the study, where walking group was supervised by instructors.

“Walking can help control blood pressure, reduce lipid or fat levels, and help with weight control — all cardiovascular risk factors,” Gordon said in the statement. “So doctors should encourage it for patients who have had a stroke.”

Larry Goldstein, MD, Director of the Duke Stroke Center, said the cardiovascular benefits of walking can help prevent another stroke.

“The better your heart is doing, the better you’re doing in general,” says Dr. Goldstein. “Beginning to walk, even long after a stroke, has been associated with functional gains.”

But he recommended that people don’t wait until they have a stroke to start walking.

“Getting regular physical exercise is one of the factors we believe is associated with a lower stroke risk,” he said.

Ultraviolet rays can literally burn your eyes, much the same way they can burn your skin. The symptoms: red, itchy, or gritty-feeling eyes. This kind of discomfort is usually short-lived, but researchers have found that long-term exposure to UV rays can have a permanent affect, including cataracts and irreversible damage to the retina. So be sure to wear sunglasses whenever you spend time outside — even if it’s cloudy. Which shades are best? The American Optometric Association recommends sunglasses that block both UVA and UVB radiation, screen out 75 to 90 percent of visible light, are free of imperfections and distortion, and have gray lenses for proper color recognition.

Consume Carotenoids

When responsibilities mount, sleep is the first thing to drop off the priority list. In fact, according to the National Sleep Foundation, as many as 47 million Americans are sleep-deprived. And that’s a problem since experts argue that getting adequate zzz’s is as important to health and well-being as diet and exercise. If you have a hard time falling asleep, wake up often in the night, or feel exhausted and doze off in the daytime, try the following tips for a more restful slumber.

Stick to a Schedule

You’ve heard it time and time again – wake up and go to bed at the same time every day, even on weekends. Why is this so important? It all relates to circadian rhythms. “Sleep is a homeostatic process [a system where our bodies regulate automatically based on our daily patterns],” says Sonia Ancoli-Israel, Ph.D., a professor of psychiatry at the University of California at San Diego and a spokesperson for the National Sleep Foundation. “If you sleep in, it might affect your ability to fall asleep the next night, since you have to be awake for a certain amount of time before you’ll be sleepy enough to go to sleep again.”

Pick a Relaxation Ritual

Engaging in a relaxing activity, preferably with dim lighting, helps separate sleeping times from times that elicit excitement, stress, or anxiety. Instead of trying to balance your budget or solve big family dilemmas right before bedtime, turn to soothing activities like taking a warm bath,meditating, or reading. It doesn’t matter what you do, claims Ancoli-Israel, as long as it relaxes you – just stay away from bright light because it signals the brain that it’s time to awaken.

Turn Everything Off

If you can view a clock from your bed, move it. In fact, according to Ancoli-Israel, getting rid of the clock is effective in 90 percent of people who have difficulty sleeping. “If you’ve just woken up and you want to know what time it is, you have to take yourself from transitional sleep to full awakening – and you’ve then made it harder to fall back to sleep,” says Ancoli-Israel. “Get rid of the clock and don’t even open your eyes if you awaken in the middle of the night, because that will take you out of that transitional sleep.”

Create a Safe Haven

Sleeping soundly requires the right environment – dark, quiet, comfortable, and cool. It helps if distractions are minimal, too, including exposure to light, uncomfortable temperatures, or poor air circulation, not to mention a spouse’s loudsnoring. While some potential sleep saboteurs are beyond your control, blackout shades, earplugs, humidifiers, and fans can help block out the major offenders.

Get Comfy

When it comes to sleep, comfort is key. If you have been lying on the same mattress for years on end, chances are it may have exceeded its life expectancy (most “good” mattresses survive for about 10 years). Make sure your mattress is comfortable you’re your sheets made from a high thread count. Encasing yourself in comfort will make for more peaceful slumber. Ancoli-Israel points out that brands and prices of these items — whether sheets, pillows, other bedding, or a mattress — are irrelevant. It’s how they make you feel that matters most.

Watch What (and When) You Eat

Don’t eat anything two to three hours before your regular bedtime. The same rule applies to liquids. You don’t want to disrupt your slumber because of the need for a bathroom trip. Avoid eating a heavy meal too close to bedtime, or spicy foods, which may cause heartburn and make it difficult to fall asleep. But you don’t want to go to bed hungry, either. “Again, it’s doing what makes you feel comfortable,” says Ancoli-Israel. Then you’ll rest easy.

Fit in Fitness

Just don’t do it right before bedtime. Study after study has shown that people who are more physically active get better sleep. “When you exercise, body temperature goes up, and it takes about six hours for it to drop again,” says Ancoli-Israel. And since a cooler body temperature is associated with the onset of sleep, an ideal time for exercise is the late afternoon.

Set the Stage for Sleep

Keep work, computers, TVs, and other distractions out of the bedroom. Reserving the bedroom for sleep and sex actually helps strengthen the association between bed and sleep. Reading in bed, for example, is fine if it helps you fall asleep. “It’s very individual,” says Ancoli-Israel. “What works for one person may not work for another.” She even suggests taking sex out of the bedroom if it isn’t relaxing for you and doesn’t promote sleep. Bottom line: When you’re in the bedroom, engage in activities that help you relax.

Use Your Imagination

Your typical waking and sleeping times are programmed in your subconscious mind. Instead of counting sheep, try resetting the program. “Both imagery and hypnosis bring your brain into a deeply relaxed state,” explains Donna Fremon-Powell, aertified guided imagery therapist, a certified hypnotherapist. “In this alpha-brainwave state, the subconscious mind is more willing to accept beneficial suggestions, such as ‘You sleep soundly through the night and wake fully refreshed and alert in the morning.'” Hypnosis and guided imagery – even listening to an imagery CD as you fall asleep – can help you change negative sleeping patterns and achieve more restful slumber.

Say No to Smoking

If you need one more reason to stop smoking, here it is: Nicotine disrupts sleep. So too, do caffeine and alcohol – and the former lurks in more than just your morning cup of joe. Tea, soda, and chocolate all contain caffeine, and they stay in the body for three to five hours. “People also need to be careful about what medications they’re taking, whether they’re over-the-counter or prescription medications,” claims Ancoli-Israel. Beyond the caffeine contained in some of these drugs is the fact that drugs, by definition, activate and mobilize your system – and that makes it harder for you to fall asleep. Take medications and herbal supplements or other remedies early in the day, and don’t drink in an effort to fall asleep. Alcohol may make you feel sleepy initially, but it actually interferes with restful sleep.

Enjoy the Daylight

The body’s natural sleep hormone,melatonin, is secreted in darkness and inhibited in light. So getting sufficient exposure to light during the day can help you stay awake and alert. By the same token, keeping your bedroom as dark as possible at night can help promote the production of melatonin and the onset of sleep. And while you can buy melatonin over the counter as a supplement, Ancoli-Israel claims it isn’t the same as the melatonin produced by the brain. Still, 1 to 3 mg of melatonin taken half an hour before bed has proven helpful for some individuals, but Ancoli-Israel claims there’s no scientific evidence to support the use of melatonin as a sleep aid.

Write It Down

Instead of ruminating over the day’s dramas right before bed, set aside a worry time earlier in the day, suggests Ancoli-Israel. “It sounds silly, but if you take 10 or 15 minutes to sit and worry during the day (with the Blackberry and beeper off), it frees you from having to think about those concerns when you get in bed at night.” If your sleep problems persist, keep a sleep diary, noting the type of problems you’re experiencing and when they occur. With early diagnosis, the outlook is good for people with chondrosarcoma, a type of bone cancer that affects cartilage.

Chondrosarcoma accounts for about a quarter of all bone cancers, which makes it the second most common type of bone cancer. It invades the cartilage, the connective tissue that prevents bones from grinding against one another and forms parts of the body like the nose and the ears.

Chondrosarcoma can occur anywhere in the body where there is cartilage. However, it most often attacks the cartilage cells of the thigh bone, arm, pelvis, knee, or spine. Other places where this type of bone cancer sometimes occurs include the larynx, trachea, chest wall, ribs, shoulder blade, or skull.

People under age 20 rarely have chondrosarcoma. It is most common in people between the ages of 50 and 70.

Chondrosarcoma: Causes
Chondrosarcoma begins with a single abnormal cartilage cell that starts dividing out of control. No one is certain what exactly prompts chondrosarcoma, although it is suspected that genetic abnormalities or damaged chromosomes might have something to do with it. The reason for this suspicion is that chondrosarcoma sometimes occurs in people with inherited diseases like Ollier’s disease or Maffuci’s syndrome, which are types of benign cartilage tumors that usually affect the hands.

Some researchers believe there also is a link between chondrosarcoma and repeated trauma to the site of the bone cancer, but it’s not clear whether the injury makes the cancer more likely or the cancer makes the site weaker and more susceptible to injury.

Chondrosarcoma: Symptoms
Some symptoms of chondrosarcoma include:

  • Sharp or dull pain where the tumor is located. The pain usually is worse at night, and will become more constant as the bone cancer grows. The pain may increase with exercise, physical activity, or heavy lifting.
  • Swelling or redness at the tumor site.
  • A large lump at the site.
  • Limping or decreased use of the affected limb.

Chondrosarcoma: Diagnosis
Diagnosis of chondrosarcoma involves a several step process that may include:

  • Imaging studies of the suspected tumor: These include X-ray, MRI (magnetic resonance imaging), and CT (computed tomography) scans. Possible areas where the bone cancer might have spread will also be examined.
  • Blood tests: These would include a complete blood cell count s and analysis of hormones and other blood chemistry that could indicate the presence of cancer.
  • Biopsy: Here, a small tissue sample is removed to be examined under a microscope. This test provides definitive proof of cancer and also allows doctors to distinguish chondrosarcoma from other bone cancers.

Chondrosarcoma: Treatment
Once chondrosarcoma has been diagnosed, there are several options available for treatment, including:

  • Bone cancer surgery is the main form of treatment for chondrosarcoma. If the tumor is in a limb, doctors will try to remove it without removing the limb. However, amputation may be necessary.
  • Radiation therapy could be used after surgery to kill off any remaining cancer cells.
  • Bone cancer chemotherapy is rarely used to treat chondrosarcoma. However, it may be used if the bone cancer has spread to other parts of the body.

If chondrosarcoma is caught before it has spread, patients have a five-year survival rate of 90 percent. However, if the bone cancer has spread to other organs or is more aggressive, the survival rate drops to 43 percent or lower.

Researchers report that fats from fish oil prevent the flu from killing mice when given with antiviral drugs, even when the mice are suffering from advanced stages of the disease.

A compound derived from fish oil helped prevent death in mice infected with the flu, even when the disease had progressed to an advanced stage before treatment, according to study published today in Cell.

Researchers reported that the PD1, derived from fish oil, was effective at stopping the flu virus from replicating in the laboratory in human lung cells. And flu-infected mice who were given PD1 in combination with an antiviral drug were more likely to survive the disease — even when the treatment was administered more than two days after the mice came down with the flu.

Antiviral drugs on the market, such as Tamilflu, are typically not effective when given to human patients after more than two days.

“Tamiflu is the most commonly used antiviral medication right now, and it does have some benefit, but the benefit is incomplete,” said Dr. Bruce Hirsch, attending physician in infectious diseases at North Shore University Hospital in Manhasset, New York.

Why We Need More Targeted Flu Treatments

The development of a more targeted flu drug that could be effective in severe flu cases has the potential to save many lives, especially for seniors or those with weakened immune systems, said Hirsch. This could be particularly helpful in severe flu seasons like the current one. The U.S. Center’s for Disease Control and Prevention (CDC) reported the flu vaccine fell short for seniors this year, providing only 9 percent protection against the nastiest strain of the virus. Overall, the CDC says the vaccine was only about 56 percent effective.

“The average efficacy of the vaccine is 60 to 70 percent in the population, so it’s imperfect, although it is helpful and people should continue to get the flu shots, especially healthy people who are around others in a weakened condition,” said Hirsch.

Flu seasons vary from year to year, and so does the hospitalization and death toll. The CDC estimates more than 200,000 people in the United States are hospitalized for flu-related complications every year, and the flu can kill up to almost 50,000 people every year.

“Flu and related complications are a dangerous problem,” said Hirsch. “The vaccine strategy is helpful, but it’s not efficient.”

According to Hirsch, the question for for future research is whether or not a person can safeguard him or herself from the flu by adding dietary omega-3 fatty acids.

Alcoholic beverages were listed as a known human carcinogen by the U.S. Department of Health and Human Services in 1997, and numerous studies on a variety of cancers have shown an alcohol-cancer link. A new study, published in the April 2013 issue of theAmerican Journal of Public Health, finds an even stronger link between cancer and alcohol deaths. Researchers from the Boston University schools of Medicine and Public Health found that alcohol resulted in about 20,000 cancer deaths in 2009, accounting for about 3.5 percent of all cancer deaths in the United States that year.

The researchers also found that each alcohol-related cancer death accounted for an average of 18 years of potential life lost in those who died. The new data serves as a reminder that alcohol is a carcinogen, even when consumed in moderate quantities. According to the data analysis in the study, average consumption of 1.5 drinks per day or less accounted for 30 percent of all alcohol-attributable cancer deaths. The strongest links between alcohol and cancer deaths were to breast cancer, esophageal cancer, colorectal cancers, and liver cancer.

Grilled Meat and Fish: Colorectal and Prostate Cancer Risks

Cooking beef, pork, fish, or poultry using high-temperature methods, like pan-frying or grilling over an open flame, can form chemicals called heterocyclic amines and polycyclic aromatic hydrocarbons, which have been linked to cancer in animals, according to the National Cancer Institute.  In 1999, a large-scale study published in the journal Cancer found well-done or grilled red meat was associated with an increased risk of colorectal adenoma, a precursor to colorectal cancer.

A more recent study, published in the journal Carcinogenesis, found cooking red meats at high temperatures, especially pan-frying, can increase the risk of advanced prostate cancer by as much as 40 percent.

Sunscreen: Possible Skin Cancer Risk

By now, everyone knows that ultraviolet radiation from the sun and UV lamps at fake tanning salons can cause skin cancer. But did you know the stuff you put on your skin to shield it from the sun’s rays may also increase your cancer risk by damaging cells?

A recent study by researchers at Missouri University of Science and Technology found that when exposed to sunlight, zinc oxide, which is an ingredient found in many brands of sunscreen, undergoes a chemical reaction that could release unstable molecules known as free radicals. These molecules try to bond with other molecules and in the process can damage DNA in the cells, which could increase the risk of skin cancer. The researchers said tests on a possible zinc oxide-cancer risk are still in the early stages, so they recommend continuing to wear sunscreen rather than forgoing any type of protection when you’re out in the sun.

Working the Night Shift: Possible Breast Cancer Risk

Working the night shift isn’t just harmful to your sleep schedule, it could also increase a woman’s risk of breast cancer. In 2007, shift work that involves circadian disruption was listed as a probable carcinogen by the World Health Organization’s International Agency for Research on Cancer (IARC). Epidemiological studies found that women who worked overnight as nurses and flight attendants had a higher risk of breast cancer than women who did not work at night. One possible reason is the disruption of the circadian system that is caused by exposure to light at night. “This can alter sleep-activity patterns, suppress melatonin production, and deregulate genes involved in tumor development,” according to the IARC.

Diesel Exhaust: Lung Cancer Risk

While the noxious smell of diesel exhaust may remind you of bus trips or weekend getaways, it could also increase your risk of lung cancer. The IRAC began classifying diesel engine exhaust as carcinogenic to humans in 2012 based on “sufficient evidence that exposure is associated with an increased risk for lung cancer.” A study published in the Annals Of Occupational Hygiene was based on research by the National Cancer Institute and the National Institute for Occupational Safety and Health involving more than 12,000 mine workers. Researchers found an increase in lung cancer rates among those exposed to diesel exhaust underground, with greater exposure linked to higher cancer rates. In those with the highest exposure, the study found deaths from lung cancer tripled.

It’s not about the total amount of antioxidants you consume to stave off stroke and dementia, but how you’re getting them, new research suggests.

The protective power of antioxidants against stroke and dementia may have less to do with your total dietary intake of antioxidants and more to do with the specific foods that contribute to your antioxidant level, new research suggests.

Researchers from Harvard Medical School in Boston and Erasmus Medical Center in Rotterdam in the Netherlands analyzed health and dietary information on 5,395 people aged 55 and older who were part of the long-term Rotterdam Study of medical conditions and other factors in older adults.

Study author Elizabeth E. Devore, ScD, now with Brigham and Women’s and Harvard Medical School, says she and her colleagues at Erasmus came at the Rotterdam research data with a new question: “Is it really about individual nutrients for dementia and stroke, or would it be important to look overall at the total capacity of the diet in terms of antioxidants?”

Not many studies have been done on total antioxidant level, says Dr. Devore, in part because they require a lot of information to produce, specifically, “large international tables where thousands of food products have been tested, looking at the antioxidant capacity of foods.” Researchers then use questionnaires in which participants report how much and how often they eat these foods, to determine the total amount of antioxidants they consume. Those tables, essential for this type of analysis, have only been around for a couple of years.

The findings, reported online today in the journal Neurology, show that the total level of antioxidants a person consumes does not seem to relate to their risk of having a stroke or developing dementia.

“It doesn’t surprise me that it isn’t the antioxidant level by itself,” says Joan Salge Blake, MS, RD, a clinical associate professor of nutrition at Boston University, who did not work on the study. “It’s probably a bunch of things working together.”

Antioxidants — which are found in fruits and vegetables, wine, coffee, tea, and other dietary sources — are proven to have a number of health benefits, from lowering heart disease risk to protecting eyesight and even preventing the development of certain types of cancers. Related specifically to the prevention of stroke and dementia, says Devore, a person’s overall intake of antioxidants appears to be less important than the fact that they eat certain antioxidant-rich foods, and fruits and vegetables stand out as key players.

New Explanations From Older Antioxidant Research

The authors of the new paper compared their findings to those of an Italian study that did find a relationship between total antioxidant levels and lower stroke risk. In that study, the Italian participants got most of their antioxidants from alcohol, fruits, and vegetables, compared to the Rotterdam study group, which got most of their antioxidants from coffee and tea. Here’s how Devore interprets that difference.

“Higher intake of fruits and vegetables has been associated consistently with lower stroke risk in multiple studies,” she says. “The fact that their score was so influenced by those might suggest that — to me, given the results of our study — we might just be seeing it’s the fruits and vegetables that are important.”

Although it contradicts some existing research, the new study supports a large analysis of participants in the Nurses’ Health Study, which Devore also worked on, and which found no relationship between total antioxidant level in the diet and cognitive decline in older women.

Upon finding no association between total antioxidant intake and risk of stroke and dementia, Devore and colleagues re-ran the findings of two studies with shorter follow-ups also based on data from the Rotterdam study group: One found vitamin C can lower stroke risk and another found vitamin E can reduce dementia risk. Those findings held true with the new study group as well.

The associations “were still there on the individual level,” says Devore, “suggesting it was really the specificity of antioxidants for this cohort. I think that is the takeaway — specificity rather than overall capacity of the diet. The type of antioxidant really matters.”
